Dear New Patient,

It is our pleasure to welcome you to Michigan Pain Specialists. We look forward to having you as our patient. Our goal is to decrease pain and increase function.

If you have Blue Care Network, please contact your Primary Care Physician to get your referral and authorization for your appointment. If you have Workers Comp or an Auto Accident claim you need to bring your claim information with you. Also, please have your adjuster call the office with authorization for evaluation and treatment. You can also have the information faxed to us at (734) 375-6585.

If you have had any recent MRI films, please bring them to your appointment. Also bring your insurance cards, picture identification, a list of medications and a list of fax numbers of all physicians who should receive reports of your care. All co-pays and deductibles will be collected at the time of your visit.

Please note, all new patients are required to have a urine screen upon arrival.
